All customers are required to complete an account profile.
This form requires your full legal name, cell phone and home/business phone numbers, billing and shipping address, date of birth, and driver’s license or photo ID number.
Please note that all orders must be completed by an adult over the age of 18 years.
In addition, the customer who has placed the order must be present to receive the product or shipment with their valid government identification and signature.
What classifies a valid – government identification?
The acceptable forms of identification are a driver’s license, a photo ID, or a passport.
